Fill out new patient paperwork → 2 entries
Attend your scheduled visit → 1 entry
Re-exam / progress check → 2 entries
Leave a review → 3 entries
Participate in wellness challenges → 2 entries
Complete your care plan → 4 entries
Refer a friend → 3 entries
(when they attend their first visit)Medicare beneficiaries are not eligible to participate
How It Works
Summer Grand Prize:
One $50 gas card awarded at the conclusion of the giveaway
(one grand prize winner will be randomly selected from all eligible entries received during the entire giveaway period.)
One winner will be drawn each month:
Each winner will receive a $25 gas card
Prizes
Official Giveaway Rules:
No purchase or payment is necessary to enter or win. The contest period runs from 5/12/2026 to 7/12/2026. A purchase or receipt of services does not improve chances of winning. A free entry is available upon request per drawing period by emailing DrBarry@SitkoffMobileChiro.com.
Open to the public and patients, excluding Medicare beneficiaries. Participation does not influence care, clinical recommendations, or fees. You must be at least 18 years old to enter. Void where prohibited.
Winners will be randomly selected from all eligible entries received during each monthly drawing period. One grand prize winner will be randomly selected from all eligible entries received during the entire promotion period.
Limit: up to 20 entries per participant per month.
